CICOA’s Care Managers assess personal care needs during at-home visits and coordinate the needed services into a care plan. Family members and other caregivers often participate in this planning whenever possible. Once a plan is developed, the Care Manager then arranges for the needed services to be delivered.

In-Home services include such things as:

  • adult day care
  • homemaker
  • respite care
  • attendant care
  • home health aide
  • home delivered meals
  • home modifications.

Our Care Managers provide continuous support, as they monitor the needs and progress of the client and adjust the care plan as needs change. CICOA’s Care Managers are professionals, with degrees in social work, psychology, sociology, gerontology and nursing.
