Medical Nutrition Therapy (MNT) is a therapeutic nutrition-based treatment and counseling service provided by CICOA’s Registered Dietitians. Appointments occur in-person or via telehealth and include a thorough lifestyle assessment, nutrition diagnosis, interventions, goal monitoring and evaluation to help manage chronic conditions with individualized nutrition care.

❑ Learn how to live a healthy, balanced lifestyle and manage your chronic conditions.
Eligibility
Medical Nutrition Therapy is a no-cost service to eligible beneficiaries with Part B Medicare. (Medicare advantage plans cannot be accepted for this program.) However, you must be referred by a physician and have one of the following diagnoses:
- Type 1 diabetes mellitus
- Type 2 diabetes mellitus
- Chronic kidney disease (CKD) Stage 3
- Chronic kidney disease (CKD) Stage 4
- Received a kidney transplant within last 36 months.
