The Family and Social Services Administration is launching a new online resource for individuals and families to confirm an individual’s waiting list placement for the Health and Wellness, PathWays, and Family Supports waivers. The Online HCBS Waitlist Dashboard is available at ddrsprovider.fssa.in.gov.
This resource provides an opportunity for the individual, family members, or advocates to confirm the individual has been added to the waiting list and to verify the accuracy of the individual’s contact information. If the requestor has additional questions or needs to correct the contact information, an email to FSSA can be directly sent from this site.

FAQs:
– What information do I need to have to use this site?
The requestor will need the following information for the individual:
- the last 4 digits of the individual’s social security number or the individual’s Medicaid identification number (RID),
- individual’s first name,
- last name,
- and date of birth.
– Is this individual’s information secure on this site?
Yes. To access information through this site, the requestor must submit key pieces of information into the search to gain access to the individual’s information. Detailed information on using this site is available in the guide.
– I am on the waitlist, but my information did not display. What do I do?
First, double-check that you entered the correct information. If you are still having issues, contact the Help Desk by clicking the email link on the screen.
– My name showed up on DART and under Integrated Management Portal. Why?
An individual who has contacted the Bureau of Disabilities Services (BDS) seeking services for individuals with intellectual and developmental disabilities will display under the DART heading. DART is the system used for intake for these services. An individual who is seeking the Health and Wellness or PathWays waivers will display under Integrated Management Portal. An individual may be in both groups.
