Community Health Workers (CHW): Our CHW program is our “proactive arm,” designed to meet people exactly where they are. In addition to taking referrals and doing home visits, our team is onsite at several locations throughout the area, acting as the “boots on the ground” to identify health risks before they become emergencies. By staying connected with community partners, our CHWs ensure that neighbors receive help early, preventing the “crisis mode” that often follows a health decline.
